Infusion Services - What We Do

“Our pharmacy care team model” provides for each patient to be assigned to a specific pharmacist responsible for managing his/her care for the duration of therapy. Our pharmacist will collaborate with your physician, homecare nurse, and other healthcare professionals to provide an integrated approach.

CSI support services include, but are not limited to:

24-hour clinical support
Clinical assessment and monitoring
Liaison between physician and patient
Development of pharmacy care plans individualized for each patient
Education and training for patient and caregiver
Provision of all needed supplies and equipment
Direct delivery service with patient privacy being foremost
